Behavioral Health Resource Center Intake Specialist | PRN

The BHRC Intake Coordinator provides direct support to patients through evaluation and assessment of Level of Care (LOC) needs, performs comprehensive bio-psychosocial assessments, and utilizes a triage matrix to determine immediate intervention or diversion to community resources. Works with a professional team of behavioral health staff to provide excellent care and customer service to patients served.

Responsibilities

• Prepares intake screening interviews and psychosocial assessments and diagnostic formulations for use in treatment and discharge planning, and Level of Care (LOC) considerations.
• Confers with members of the professional clinical staff throughout all patient treatment processes.
• Maintains all records and documentation according to hospital policy and procedure.
• Evaluates and recognizes any urgent crisis of patient and/or their families and facilitates appropriate necessary interventions.
• Provides consultation to BHRC staff within the scope of their practice and expertise in areas including mental health, substance use disorders, suicide assessments, trauma-informed care, crisis intervention, and disposition planning.
• Facilitates referrals needed, such as transfer to ECC for potential further evaluation, referrals for detoxification services, and all other follow-up care.
• Makes contact with referral sources and provides any needed information to ensure the best outcome for the patient.
• Provides patients and families with education, comfort, and relevant information in accordance with their assessed care needs.
• Compiles a referral source database, including ALF, nursing homes, substance abuse facilities, homeless shelters, etc., to provide the least restrictive, appropriate placement.
• Works closely with Care Connect+ staff to assess for and address any social determinants of health as well as assessing for any additional barriers to continued treatment.
• Performs all other duties as assigned by management within job scope.

Requirements

• Bachelor's Degree
• 0–1 year of experience in a mental health care environment
• Basic Life Support (BLS) certification, provided by American Heart Association (AHA), required within 30 days of employment
• Completion of WELLE training (Non-Abusive Psychological & Physical Intervention) within 90 days of employment; annual re-certification required thereafter
• Level 2 background screening

Preferences

• Mental health care experience
• Experience administering assessments and utilizing standardized assessment tools
• Ability to effectively communicate with a multidisciplinary team, establish good rapport with clients, and build relationships with partner agencies, community providers, and all internal resources
