About the role
The Mental Health Rehab Worker, under the direction of a mental health professional or practitioner, provides rehabilitative services to adults with mental illness or children with emotional disturbance.
Responsibilities
- Completes functional assessments and level of care assessments
- Provides coping skills education and modelling
- Prompting and observing clients to meet their activities of daily living
- Provides mental health services as outlined in the client’s treatment plan
- Completes and updates intake paperwork, screeners, functional assessments, releases, and forms/paperwork as needed
- Affords targeted case management as a Case Management Associate
- Completes and implements treatment plans
- Assists clients in completing structured daily living activities
- Participates in clinical supervision and team meetings
- Works collaboratively in care coordination activities
- Performs crisis assessments and interventions
- Ensures care coordination activities are client-centered
- Represents Alluma at community events
- Performs additional duties as assigned or requested
Qualifications
- High school diploma or equivalent
- Fluent in non-English language or competent in the culture of the ethnic group to which at least 20% of the mental health rehab worker clients belong
- Associate of arts degree
- Two years of full-time postsecondary education or 15 semester hours or 23 quarter hours in behavioral sciences or related fields
- Registered nurse
- Three years of personal life experience with mental illness
- Three years of life experience as a primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability
- 000 hours of work experience providing health and human services to individuals
- Passes a criminal background check through MN DHS NetStudy
- Valid driver’s license and insurable under the agency’s automobile insurance policy
- Free from substance use problems for no less than 2 years
