About the role
The behavioral health navigator level III serves as a senior member of the care coordination team, providing expert support for high-acuity behavioral health cases. This role includes workflow leadership, mentoring, and project participation to enhance service delivery across the continuum.
Responsibilities
- Provide expert support for high-acuity behavioral health cases
- Lead workflow and mentor other team members
- Participate in projects to enhance service delivery
Requirements
- Master's Degree in Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Addiction Studies, Nursing, or a closely related field
- Bachelor's of Science in Nursing for RN candidates
- RQI certification required within 90 days of hire
Qualifications
- Eight years of healthcare experience delivered in one of the following settings: Acute care, long-term care, home health, hospice, skilled nursing facility, or community-based program services
Skills
- Strong communication skills
- Ability to manage complex caseloads
- Experience with care coordination and patient advocacy
