Behavioral Health Counselor, Internal Medicine

Job Summary

In this role, the Behavioral Health Counselor will primarily support patients referred from the Saugus Internal Medicine practice, while also providing virtual coverage to the Lynn Primary Care practice as needed. Working as part of an integrated care team, the counselor will collaborate closely with primary care providers, nurses, and social work case management.

Responsibilities

  • Conducting behavioral health assessments
  • Developing treatment plans
  • Providing brief interventions
  • Supporting primary care as needed such as crisis intervention; care coordination; information; referrals; and safety planning
  • Assessing new patients to aid in the identification of the best treatment options for them
  • Participating in formal and informal case reviews, seminars and / or inter- and intra-departmental peer review meetings
  • Serving as a liaison between patients and other members of the Behavioral Health Team as well as the Primary Care and Collaborative Care Teams to coordinate services needed to improve the lives of our patients

Qualifications

  • Master's Degree in a related field of study
  • At least 3 years of related experience

Additional Job Details

  • Hybrid opportunity with onsite coverage 3 days per week and remote coverage 2 days per week
