Responsibilities

  • Lead Exceptional Care Delivery: Oversee and coordinate home health services tailored to behavioral health needs. Manage patient and provider assignments, ensure compliance with regulations, and prioritize timely starts of care.
  • Champion Quality & Compliance: Approve and evaluate individualized care plans to maintain the highest clinical standards. Seamlessly integrate and coordinate physician orders.
  • Build a Culture of Compassion: Inspire and support a team of healthcare providers, ensuring patient needs are continuously assessed and met with excellence.
  • Streamline Operations: Manage processes for physician orders, staff communication, and interdisciplinary coordination for holistic care.

Qualifications

  • Education: Associate's Degree in Nursing (Bachelor's preferred).
  • Experience: At least 2 years as a Registered Nurse in Home Health or Behavioral Health, with 1+ year in a leadership role.
  • Expertise: Familiarity with enterprise systems (Workday, HomeCare HomeBase) and MS Office Suite.
  • Licensure: Current state RN license.

Pay

Competitive pay based on experience in behavioral health, nursing, and education, with a range from $100k to $115K.
