Behavioral Health Case Manager - Outpatient

About the role
The Behavioral Health Case Manager provides comprehensive case management services, including discharge planning across Vail Health’s Behavioral Health service line and community behavioral health systems. The role aims to reduce barriers to care, enhance coordination, improve outcomes through supportive case management, address social determinants of health, and promote early intervention, risk reduction, patient education, and empowerment. This position operates within a patient-centered, strength-based, recovery-oriented, wraparound approach, partnering with behavioral health providers, agencies, and medical teams to ensure timely access to care, continuity, and client wellbeing.
Responsibilities
- Provide comprehensive case management services to ensure coordinated delivery of care for high-risk, high-acuity community members under the supervision of the Director of Case Management.
- Manage a caseload based on community and patient population needs, maintaining 32 billable hours per week.
- Maintain excellence in clinical documentation aligned with local, state, federal, and regulatory standards.
- Engage in comprehensive service plans and client assessments addressing clinical needs, level of care, and social determinants of health (SDoH).
- Complete comprehensive discharge planning for patients transitioning from higher levels of care.
- Support care coordination with behavioral health and medical providers, including home visits and assistance with appointments to ensure continuity of care.
- Develop case management care plans, set goals, and coordinate/attend care meetings with providers and community partners.
- Facilitate referrals, resources, and coordination with community agencies to support total health outcomes.
- Provide patient, family, and community-level education, prevention, and skill development to improve outcomes.
- Monitor patient outcomes, experience, and feedback, collaborating with team members to enhance care quality.
- Analyze data and evidence to inform standards, practices, and patient care models; review and update clinical policies and procedures.
- Prepare reports, maintain records, and compile data/statistics as required.
- Maintain security and confidentiality of data in compliance with hospital policies, laws, and regulations.

- Perform other duties as assigned.
Requirements
- At least one year of experience in human services, behavioral health, or a related field.
- Experience with HIPAA and privacy laws, including Part 2/42 CFR restrictions.
- Understanding of Colorado laws and regulations related to mental health practices, privacy, confidentiality, supervision, and ethical care (preferred).
- Experience with data collection, reporting, and Medicaid billing/reporting requirements (preferred).
- Direct experience working with diverse populations (preferred).
- Experience with behavioral health-specific electronic medical records, documentation standards, coding, and minimum standards of care (preferred).
- Understanding of American Psychological Association (APA) ethical codes and applicable statutes (preferred).
- Rural behavioral health experience with knowledge of Eagle County resources and community agencies (preferred).
- Bilingual skills strongly preferred.
Qualifications
- Current, valid Colorado Driver’s license with ability to pass Vail Health’s Department of Motor Vehicle Report.
- Basic Life Support (BLS) certification.
- Bachelor’s degree in social work, psychology, or a human resource-related field (preferred).

Pay
Pay is based on relevant education and experience. Yearly pay range: $63,689.60 USD – $87,880 USD.
