Location: Riverside County, CA
This is a hybrid position requiring travel throughout Riverside County up to 5 days per week. Candidates must reside within 20 miles of the assigned territory due to frequency of travel.
About the Role
The Clinical Lead Care Manager (CLCM) serves as the client’s primary point of contact, coordinating with providers such as doctors, specialists, pharmacists, and social services to align on client needs and care. The CLCM manages client cases, coordinates health care benefits, and facilitates timely access to care while collaborating with caregivers, family, and the care team to promote wellness, recovery, and independence.
Responsibilities
- Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services.
- Oversee development of client care plans and goal settings.
- Provide services where the member resides, seeks care, or finds most accessible (office-based, telehealth, or field-based).
- Connect clients to social services and supports, and advocate on their behalf with healthcare professionals.
- Utilize evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
- Conduct outreach and engagement activities; log activity in the Client Relationship Management (CRM) system.
- Evaluate client progress and update SMART goals.
- Provide mental health promotion and arrange transportation (e.g., ACCESS).
- Complete documentation, including outcome measures, within care plan timeframes.
- Maintain up-to-date patient health records in the Electronic Medical Record (EMR) system.
- Complete monthly reporting for program compliance.
- Attend assigned training.
Requirements
- Active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California.
- 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations (2+ years preferred).
- Willing and able to work Monday–Friday, 8:30 AM–5:00 PM, both in the field and remotely, with flexibility for potential evenings/weekends.
- Working knowledge of government and community resources related to social determinants of health.
- Excellent oral and written communication skills; positive interpersonal skills.
- Valid driver’s license and reliable transportation.
- General computer skills and working knowledge of Google Workspace, MS Office, and the internet.
- Bilingual (English/Spanish) required.
