Behavioral Health Case Manager

Onsite in Jackson Heights, NY – Full-time, Monday–Friday, 9:00 AM–5:30 PM.

About the role

This position involves providing behavioral health case management services in a primary care clinic setting. The successful candidate will use various technology platforms to manage patient records and coordinate care.

Responsibilities

• Provide behavioral health case management services.
• Coordinate and link patients with additional services.
• Maintain accurate patient records using technology platforms.
• Perform crisis intervention and counseling.

Requirements

• Advanced degree in mental health counseling or social work.
• Two years of work experience in a related field.
• Licensure with LMHC or LMSW within three months of hire.
• UAS-NY certification within one month of hire.
• Experience working with diverse populations.
• Excellent written and verbal communication skills in English.
• Ability to work flexible hours including some evenings and weekends.

Qualifications

• Proven experience in behavioral health case management.
• Strong crisis intervention and counseling skills.
• Proficiency in using various technology applications.
• Fluency in Spanish or an Asian language is a plus.
• Excellent customer service skills.

Pay

$68,140.80–$71,198.40 per year.

Schedule

Monday–Friday, 9:00 AM–5:30 PM, with occasional evenings and weekends.
