About the role
We are seeking a Behavioral Health Case Manager to join an integrated care team, working approximately 20 hours per week in a hybrid setting with scheduled onsite days in Engelhard and Manteo.
Responsibilities
- Managing referrals from primary care and behavioral health providers
- Coordinating comprehensive care plans
- Connecting patients to specialty and community resources
- Addressing social determinants of health such as housing, transportation, and food access
- Collaborating closely with medical and behavioral health professionals
- Supporting patient advocacy
- Maintaining accurate documentation in the electronic health record system, preferably Athena
Qualifications
- Bachelor's degree in social work or related field
- Experience in case management preferred
- Candidates pursuing a degree may be considered
