Job Description
The Behavioral Health Case Manager (BHCM) serves as a member of a multi-disciplinary patient care team for NATIVE HEALTH’S Behavioral Health Department. The BHCM provides case management services that support the treatment goals of the patient's care plan. The BHCM represents the company in a professional and positive manner to enhance and promote the core values and mission of NATIVE HEALTH, always exercising the utmost discretion, diplomacy, and tact in customer interactions.
The Behavioral Health Case Manager reports directly to the Behavioral Health Coordinator. KEY ACCOUNTABILITIES:
- Provides non-intensive case management services in support of Behavioral Health patients and providers.
- Supports the behavioral health program and its team members.
- Engages in quality improvement and quality assurance activities for the department.
- Supports the organization as needed.
Qualifications
- Bachelor’s degree in social work or a related behavioral health field is required.
- A master’s degree in social work or a related field is preferred.
- Two years of relevant work experience.
- Bilingual in Spanish is highly preferred.
- Experience working with underserved and diverse populations is preferred.

- Knowledge of existing social and behavioral health resources for patient care.
- Excellent written and verbal communication skills and ability to establish rapport.
- Good documentation skills, including detailed and accurate documentation.
- Knowledge and understanding of mental health/substance abuse terminology.
- Knowledge of the mental health issues of the populations served.
- Ability to work independently with a strong sense of focus, task-oriented, non-judgmental, and a clear sense of boundaries.
- A strong sense of and respect for confidentiality.
- Courteous, honest, and professional always.
- Customer service oriented.
- Ability to plan work and execute assignments effectively.
- Excellent computer skills with Microsoft Office and electronic health records.
- Remain current and complete all assigned Relias trainings on time.
Starting Hourly Pay
$23.51
