The Behavioral Health Case Manager (BHCM) serves as a point of contact and advocate to members. They provide face-to-face, phone, virtual, and in-home services as needed by each member's journey to recovery. They develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. Serve as a point of contact and advocate as needed, to ensure ongoing collaboration, including the communication of appropriate clinical information regarding progress of the person served with other individuals and/or entities with whom delivery and coordination of covered services is important (e.g., primary care providers, adult probation, other involved service providers).

Minimum Qualifications

  • Bachelor's or Master’s degree, no experience necessary
  • Associate degree plus one (1) year* behavioral health work experience
  • High School Diploma or GED plus Peer Support Specialist certification plus two (2) years* of behavioral health work experience
  • High School Diploma or GED plus Medical Assistant certification plus two (2) years* of behavioral health work experience
  • High School Diploma or GED plus three (3) years* of behavioral health work experience.

Qualified Applicants Must be able to pass a pre-employment drug screen and background check. Additional Requirements

  • Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.

Because this is a safety sensitive position and in accordance with its Drug- and Alcohol-Free Workplace policy, COPE reserves the right to exclude or remove a person from this position if COPE believes in good faith that the employee's current use of any drug, legal, prescribed by a physician or otherwise, if the drug could cause an impairment or otherwise decrease or lessen the employee's job performance or ability to perform the job duties.
