About the Role

A Care Management Coordinator is responsible for coordinating care for Medicaid members within a regional, field-based setting.

Responsibilities

• Evaluate member needs and develop personalized care plans.
• Partner with multidisciplinary teams to deliver coordinated behavioral health services.
• Ensure timely access to necessary healthcare services.
• Travel within the designated region up to 50% of the time to facilitate member care.

Requirements

• Bachelor's degree or non-licensed master's in behavioral health.
• Strong interpersonal and coordination skills.
• Ability to work independently and collaboratively with teams.
• Excellent organizational and communication abilities.
