Description
The Behavioral Health Care Manager (BHCM) will support the coordinated management of psychiatric conditions within a primary care setting. Key responsibilities include:
- Screens and assesses patients for common mental health and substance abuse disorders.
- Facilitates patient engagement and follow-up care.
- Tracks treatment response and monitors patients for changes in clinical symptoms and treatment side effects or complications.
- Documents in-person and telephone encounters in the registry and identifies/re-engages patients.
- Provides brief psychotherapeutic interventions using evidence-based techniques.
- Supports psychotropic medication management.
- Develops and completes relapse prevention self-management plans.
- Facilitates referrals for evidence-based psychosocial treatments and outside services.
- Consults with medical providers and psychiatric consultants to make treatment plan adjustments.
- Participates in regular caseload consultations with a psychiatric consultant.
- Develops skills in population-based health management and clinical research.
Minimum Qualifications
- Master's Degree in social work, counseling, or equivalent.
- At least 2 years' experience in providing psychotherapy or counseling in a mental health or primary care setting.
- Experience with primary care-based behavioral health or similar collaborative care models.
- Interest/background in health psychology.
- Experience with addiction counseling or chemical dependency treatment preferred.
- Current licensure to provide therapy or counseling services in the state of Georgia (LMHC, LMFC, LCSW, LPC, PsyD or related field).
- Basic Life Support certification (includes CPR) or obtain within 30 days.
Physical Requirements
Moderate physical demands: 20-50 lbs occasionally, 11-25 lbs frequently, 01-10 lbs constantly. Requires occasional to frequent standing, walking, close eye work (computers, typing, reading, writing), lifting up to 50 lbs, carrying up to 25 lbs, and use of personal protective equipment.
Environmental Factors
Environmental factors may vary based on assigned work area and tasks. May include blood-borne pathogen exposure, bio-hazardous waste handling, chemical exposure, communicable diseases, electrical shock, floor surfaces, hot/cold temperatures, indoor/outdoor conditions, latex allergies, lighting, patient care/handling injuries, radiation, travel, and shift work.
