Bed Control Coordinator (Full-Time) - Care Coordination Centeration

Responsibilities

• Ensures accurate and timely patient assignment and transportation based on needed service(s), level of care and equipment.
• Coordinates transportation of patients, including alternative modes as needed.
• Completes daily checks for pre-scheduled trips to confirm appointment and pick up times.
• Collaborates with Transfer Coordinator to manage patient admissions, transfers and placement within the capacity and capabilities of each facility/unit.
• Utilizes appropriate resources for transport, such as Heart Reach, air medical, contracted providers.
• Facilitates safe and efficient patient throughput by placing the right patient in the right bed at the right time.
• Coordinates and strategizes the complex placement of incoming admissions, inter-hospital transfers, and post-operative patients in accordance with hospital policies, using EPIC Bed Planning software.
• Works with Transfer Coordinator to manage patient admissions, transfers and placement within the capacity and capabilities of each facility/unit.
• Utilizes his/her knowledge and expertise to facilitate safe and efficient patient throughput focusing on placing the right patient in the right bed at the right time.

Qualifications

• Minimum of 1 year healthcare experience or training.
