Bachelor Level Intensive Care Coordinator

About the role

Responsibilities

  • Provide comprehensive care coordination to children and families.
  • Perform initial clinical assessments to identify family and youth's need and goals.
  • Develop, implement and monitor individualized care plans based on family needs and goals.
  • Facilitate care plan team meetings.
  • Act as a liaison with collateral agencies and programs.
  • Advocate for families and educate them on self-advocacy.
  • Identify and assist youth and families in accessing necessary services.
  • Ensure proper documentation, and safety planning.
  • Participate in staff meetings, supervision, and training.

Requirements

  • Excellent communication and organizational skills.
  • Basic computer fluency, including Microsoft Office.
  • Valid driver’s license and reliable vehicle.

Qualifications

  • Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university.
  • Minimum of one-year experience working with children with serious emotional disturbance.
  • Community-based experience strongly preferred.

Skills

  • Excellent communication and organizational skills.
  • Basic computer fluency, including Microsoft Office.
  • Valid driver’s license and reliable vehicle.

$22.28/hour

Schedule

Full Time 40 hours per week
