Bachelor Level Clinical Coordinator/Discharge Planner Inpatient Redwood Full Time

As a Bachelor's Level Clinical Coordinator at Pine Rest, you’ll be part of a tight-knit team that believes in the healing power of your expertise and compassion. Bachelor’s level Clinical Coordinators are responsible for upholding the healing ministry of Christ through relationships that honor integrity, stewardship, empowerment, the promotion of diversity, and professional excellence.
Responsibilities
- Work collaboratively with other community agencies on behalf of clients and/or the organization.
- Professionally and calmly handle crisis calls, responding appropriately to the acuity and needs of internal and external customers.
- Coordinate workflow and participate effectively and collaboratively in a multi-disciplinary team to ensure excellent patient experience.
- Establish therapeutic relationships with assigned patients/clients including engaging, de-escalating, screening, and managing crises.
- Serve as a liaison between community resources, payers, family, patient/client, and hospital- or clinic-based providers of care.
- Assess patient needs and refer for determination of appropriate level of care, handle insurance information, gather relevant information for clinicians, maintain patient information, and coordinate discharge planning and aftercare.
- Document and report all necessary information according to established policies and procedures when applicable.
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field required.
- A minimum of one year of related work experience preferred to direct workflow and effectively prioritize workload.

Pay
The equivalent base pay range for this position is $19.76 - $29.64 per hour. Your interview and offer letter will specify whether the role is hourly or salaried. This range does not include potential shift differentials, incentives, or bonuses, which may be part of the total compensation package. Final base pay will be determined based on location, experience, education, certifications, and FTE status.
