About the role
JAG Physical Therapy is seeking an experienced Orthopedic At-Home Physical Therapist to provide care in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic conditions. The ideal candidate will have a strong background in orthopedic rehabilitation and a passion for helping patients regain mobility and function.
Responsibilities
- Educate patients on safe bed mobility, transfers, and ambulation in consideration of post-operative precautions.
- Transfer, ambulate, and perform stair negotiation with patients to ensure they are safe for discharge to home.
- Collect and record patient care data following department policies and standards of practice.
- Prepare for evaluations by obtaining patient history, researching diagnoses, and administering tests.
- Synthesize evaluation results to formulate a diagnosis, goals, prognosis, and treatment plan.
- Collaborate with interdisciplinary teams and patients' support systems to modify and adjust the plan of care as needed.
- Provide education and training to patients and their support systems on functional training, ADLs/IADLs, and community reintegration.
- Treat patients safely, effectively, and in compliance with JAG's protocols.
- Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received.
Requirements
- Bachelor’s or Master’s Degree in Physical Therapy, with a Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in appropriate state (NY, NJ).
- Valid CPR License.
- Experience/knowledge with EMR software is preferred.
- Excellent oral and written communication skills.
- Strong clinical decision-making skills.
- Excellent work ethic and dedication to patient success.
- Basic computer skills.
- Works as a team member and individually with minimal supervision.
- Maintains a positive, professional demeanor at all times.
Qualifications
