At Home Physical Therapist

About the role

We are seeking a compassionate Orthopedic At-Home Physical Therapist to evaluate and treat patients recovering from surgeries and orthopedic conditions in their homes.

Responsibilities

• Conducting evaluations
• Developing and implementing individualized treatment plans
• Providing patient education on mobility, transfers, and safety precautions
• Assessing patient progress, documenting care using EMR software, and collaborating with an interdisciplinary team
• Performing functional training, including ADLs/IADLs, and ensuring safe community reintegration

Qualifications

• Master's or Bachelor's degree (Doctorate preferred)
• Licensed in the practicing state
• Experience in orthopedic rehab
