Job Summary
This position is responsible for supervising the direct care staff, coordinating all personal care services provided to residents by direct care staff, promoting the health and wellbeing of each resident, and reporting observed changes in resident condition or health to the Director of Health and Wellness and resident’s responsible party.
Essential Functions
- Performs thorough health evaluation on all new residents upon move-in to the community.
- Maintains oversight of the health status of all current residents through evaluations and care conferences scheduled quarterly or more frequently as required.
- Maintains compliance with service plans for team members, ensuring adherence to medication side effects, programming, meals, complaints, cognition and behavioral expressions, ADLs, and special requests.
- Reads all alert charting from daily reports and initiates actions as required to improve resident's health status.
- Coordinates and attends resident care conferences and service plan reviews with a focus on assuring service plans meet resident needs.
- Responds immediately to suspected resident abuse, conducting thorough investigations and notifying appropriate parties.
- Supervises all Assisted Living and Memory Support personnel, including hiring, coaching, orientation, scheduling, and training of staff.
- Provides evaluations and communicates employee relations issues to the Executive Director.
- Carries out necessary verbal and written disciplinary actions, ensuring documentation meets Company policies and procedures.
- Attends daily management meetings, staff meetings, and other meetings as delegated by the Executive Director.
- Collaborates with the Executive Director and The Support Center to conduct community audits, reviewing swing and night shifts for staff compliance and community security.
- Reviews all incident reports, conducts investigations, and reports or plans corrective action as required.
- Forwards all incidents to the Regional Director, Health and Wellness.
- Works with the Executive Director to ensure Continuous Quality Improvement (CQI) activities within the community meet quarterly meeting goals and documents plans to improve quality.
- Supports the Executive Director in performing clinical audits as required and requested by the management company.
- Maintains compliance with local, state, federal, and licensing rules and regulations associated with the licensure of the building, including HIPAA, OSHA, and State Survey issues.
- Ensures all employees work in a safe manner and manages unsafe actions.
- Complies with Safety Committee Standards.
- Manages workplace injuries by following procedures for treatment, investigation, return to work, and management of the injured worker, communication with care providers, and the management company.
- Fosters teamwork with co-workers, showing initiative in overcoming obstacles, taking actions that assist others beyond job responsibilities, and having a positive impact on co-workers, residents, and visitors.

- Supports the Executive Director in maintaining compliance with all local, state, federal, and licensing rules and regulations associated with the licensure of the building as they pertain to clinical and risk management activities.
