Pacific Health is expanding its team and seeking a highly skilled, compassionate therapist for a hybrid role. This position involves conducting comprehensive assessments, developing individualized treatment plans, delivering evidence-based interventions, and maintaining accurate clinical documentation. The therapist will play a vital role in helping clients achieve their mental health goals and improve their overall well-being while maintaining the confidentiality of client information.
Responsibilities
- Deliver direct and telehealth therapy services to clients via phone, video, and in-person.
- Provide crisis intervention and short- and long-term psychotherapy.
- Conduct mental health evaluations and formulate treatment plans.
- Offer referrals and case management services.
- Complete in-person clinical assessments with new Clubhouse members as part of the enrollment process for Clubhouse membership.
- Collaborate with clubhouse staff to address the unique needs and challenges of clubhouse members.
- Maintain up-to-date, accurate clinical records in compliance with HIPAA.
- Ensure all documentation is completed within established deadlines.
- Adhere to mandated reporting protocols.
- Engage with case management, administrative, and leadership teams to ensure coordinated care.
- Maintain professional, timely communication via email, phone, and instant messaging.
- Participate in team meetings, trainings, and interdisciplinary planning.
- Establish positive, trusting rapport with patients.
- Interview and conduct diagnostic assessments of patients to develop treatment and recovery plans.
- Participate in the intake process by interviewing patients and/or their family members to obtain relevant history.
- Provide ongoing clinical assessment, treatment planning, and clinical treatment for patients and families.
- Prepare and maintain clinical documentation while adhering to HIPAA regulations.
- Complete administrative requirements, including timely documentation by set deadlines.
- Collaborate with the Enhanced Care Management (ECM) team on comprehensive case conceptualizations to optimize member outcomes.
- Work independently with minimal supervision.
- Demonstrate punctuality, reliability, and responsiveness.
- Uphold a high standard of customer service and empathy.
Requirements
- Master's degree from an accredited college or university in psychology, marriage and family therapy, social work, or counseling.
- Current California license as one of the following:
- Associate Marriage and Family Therapist (AMFT)
- Associate Clinical Social Worker (ACSW)
- Associate Professional Clinical Counselor (APCC)
- Previous experience working with mental health consumers; case management experience is preferred.
- Fluent in written and spoken English; bilingual in Spanish preferred.
- Proficient in Google Suite (Docs, Sheets, Slides) and Gmail.
- Strong written and verbal communication, analytical, and multitasking abilities.
- Compassionate and empathetic approach to patient care.
- Collaborative spirit and professional demeanor.

Pay
$30.00 - $35.00 per hour
Schedule
Full-time; 9am - 6pm PST. Hybrid, 1 day on-site at locations in San Diego County.
