Associate Director, Advanced Practice Provider

Role Summary

As the Associate Director, APP Lead, you will serve in a dual clinical and leadership role. You will dedicate approximately 50% of your time to direct patient care and panel management. You will devote the remaining 50% to provider leadership, performance management, operational oversight, and clinical excellence. You will lead a team of Advanced Practice Providers (APPs) within CenterWell Medical Group's virtual care model, driving quality outcomes, patient experience, provider engagement, and operational performance. Working with physician leaders, operational leaders, and interdisciplinary teams, you will help ensure the delivery of high-quality, value-based virtual care. This is a remote position reporting to our Director of Physician Strategy for Primary Care.

What It Takes To Succeed

To be successful in this role, you will have experience managing chronic and complex patient populations, expertise in virtual care delivery, and skills necessary to lead geographically dispersed provider teams. You will bring strong clinical leadership, provider coaching, and population health management skills, along with an understanding of value-based care. To be successful, you will excel at driving quality, patient safety, operational performance, and continuous improvement while promoting collaboration and accountability across teams.

Direct Patient Care (50%)

  • Provide comprehensive virtual primary care and coordinate interdisciplinary care
  • Support achievement of quality, Stars, HEDIS, risk adjustment, and value-based care outcomes
  • Maintain high patient satisfaction and engagement
  • Ensure accurate, compliant documentation

APP Leadership & Team Management (50%)

Provider Leadership & Engagement

  • Lead a multi-state team of APPs
  • Serve as the primary contact for assigned APPs, promoting accountability, collaboration, and professional growth
  • Promote provider engagement, retention, development, and onboarding

Performance Management

  • Conduct performance reviews and development planning
  • Monitor clinical, operational, and quality performance metrics
  • Recognize achievements, promote best practices, and address performance gaps
  • Partner with physician and operational leaders on corrective action planning

Operational Oversight

  • Manage provider scheduling, PTO, and coverage planning
  • Partner with leadership to improve access, capacity, and patient experience
  • Monitor performance metrics and implement workflow improvements

Quality & Clinical Excellence

  • Promote evidence-based care, quality outcomes, and regulatory compliance
  • Monitor quality outcomes and support continuous improvement projects
  • Partner with physician leaders to improve care models and best practices

Strategic & Program Development

  • Contribute to the evolution of CenterWell Medical Group's virtual care model through participation in multidisciplinary committees and workgroups
  • Provide clinical insight and help implement new programs, workflows, and quality projects to enhance care delivery, provider experience, and patient outcomes

Required Qualifications

  • Current, unrestricted Nurse Practitioner or Physician Assistant license
  • Active, unrestricted NP license in 2-3 or more of the following states: FL, TX, AZ, GA, LA, NC, SC, MS, VA prior to the start of employment, with an ability to obtain additional state licensure if needed
  • National board certification
  • DEA registration and prescriptive authority
  • 5 or more years of clinical experience
  • 2 or more years of provider leadership, mentorship, or supervisory experience

Preferred Qualifications

  • Experience delivering virtual care in primary care, internal medicine, or geriatrics settings with an understanding of value-based care, Medicare Advantage, and senior patient populations
  • Experience managing providers using productivity, quality, and population health metrics
  • Working knowledge of Stars, HEDIS, risk adjustment, and population health strategies

Work at Home Requirements

To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel

While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay

$115,200 - $158,400 per year. This job is eligible for a bonus incentive plan based upon company and/or individual performance.
