The Assisted Living & Wellness Director (LPN required) is responsible for creating an atmosphere that provides for continuity of quality care and quality of life for all residents in the Assisted Living and Independent Living areas 24 hours per day. This is accomplished by providing clear direction and expectations to all partners about the philosophy of the area in which they work.

Responsibilities

  • Oversees daily operations of the Assisted Living including supervision of staff.
  • Completes Assisted Living 90-day and Annual evaluations.
  • Plans, evaluates, and takes responsibility for providing consistent and sufficient staffing to meet resident needs in the living areas.
  • Participates in recruitment and retention of partners.
  • Assists partners in utilizing appropriate techniques for effective assessment, planning, intervention, and evaluation of residents and their needs.
  • Sets productivity expectations and holds partners accountable for meeting them.
  • Provides wellness services as indicated.
  • Ensures that the medical, psychosocial, emotional, and spiritual needs of residents are addressed, followed up in the Plan of Care in an appropriate and supportive manner.
  • Ensures the area and its equipment and supplies are neat, clean, safe, and utilized appropriately at all times.
  • Ensures campus and department policies, procedures, and guidelines, as well as local and state regulations, are met and followed by everyone in the area.
  • Ensures adequate orientation is provided to all partners in the area.
  • Participates with the interdisciplinary team by listening to concerns and suggestions and by providing follow-up and feedback in a timely manner.
  • Coaches and teaches partners how to communicate their needs as well as resident needs.
  • Ensures exceptional customer service is provided.
  • Ensures the provision of resident physicals, drug screens, and TB screens.
  • Schedules CPR and First Aid training for partners, tracks attendance to ensure compliance.
  • Assists with partner scheduling and staffs partners according to caseload.
  • Provides work assignments to nurses and aides.
  • Ensures partners complete assignments and schedules.
  • Conducts new resident assessments.
  • Attends resident care conference meetings.
  • Updates incident reports to include interventions, writes progress notes reflecting interventions, enacts interventions, and signs incident reports.
  • Coordinates transitions in care from independent living to assisted living to long-term care.
  • Attends meetings as scheduled.
  • Attends daily morning health services leadership meetings.
  • Reports including details and analysis of any unplanned hospitalizations, ER visits, and falls.
  • Reviews progress notes and reports relevant information to the team.
  • Conducts daily partner huddles to relay pertinent, real-time information to staff.
  • Meets with Director of Health Services weekly to establish common goals and evaluate progress toward set objectives.
  • Completes shepherd visits weekly.
  • Serves as on-call nurse in rotation with the Director of Nursing.
  • Conducts periodic staff meetings to receive feedback from partners.
  • Conducts periodic evening, night, and weekend visits to stay connected to partners who work at these times.
  • Develops processes and procedures for assisted living & wellness as needs arise.
  • Communicates and builds positive relationships with family members.
  • Assists in the referral management process.
  • Provides hands-on assistance in working the floor during periods of staffing challenges.

Requirements

  • Graduate of an approved nursing program (LPN or RN).
  • Current Ohio Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Strong organizational skills and system development.
  • Effective communication and exceptional customer service.
  • Commitment to quality care and alignment with Otterbein Marblehead's mission and values.
  • Previous experience in a continuing care community preferred.
  • 2 years of healthcare experience preferred.
