Assisted Living LPN Nurse Manager- Full Time Evening Shifts

Position Summary

Supervise Resident Care and Medication Assistants, ensuring required support is provided to residents according to their Individual Service Plan, completing day to day procedures, ensuring resident safety and that all care needs are met according to the Department of Health regulations and Glenmere policy.

Essential Job Functions

  • Assisting with planning, developing, organizing implementing, evaluating and supervising resident care services, including assistance with medication in accordance with current rules, DOH regulations and guidelines that govern the community.
  • Ensures RCAs are aware of job duties and supervise the completion of all duties.
  • Mets with administration, medical and Resident care staff, as well as other related departments in planning resident care.
  • Interprets care policies and procedures to staff, residents, family members, etc., as necessary.
  • Maintains service records, resident files, and progress notes as required.
  • Makes written and oral reports or recommendations to the Nurse Manager, as necessary or required, concerning care services.
  • Maintains an adequate liaison with families and residents.
  • Notes and reports errors in the medications supervision program.
  • Reports any complaints or grievances made by residents to the Nurse Manager.
  • Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.
  • Provides guidance and training for new and/or less experienced staff.
  • Makes appropriate reports to the Nurse Manager as required or as may be necessary, including departmental performance evaluations.
  • Aids in identifying and correcting problem areas and/or the improvement of services.
  • Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to the Nurse Manager.
  • Reviews resident care needs and physicians' orders throughout each shift.
  • Assists residents with ADL’s as necessary and encourage Resident to participate.
  • Attends and participates in workshops, seminars, and in-services as required to keep abreast of current changes in the health care field.
  • Ensures that staff, residents, visitors, etc., follow established safety policies and procedures at all times.
  • Encourages residents, as necessary, to participate in ADLs based on their level of care (ALR, EALR, SNALR).
  • Ensures that staff follows established infection control procedures when performing duties and adequate supplies are available.
  • Ensures that all staff are knowledgeable and follow residents' rights and HIPAA guidelines of Resident’s health information.
  • Assists in residents with prescribed injectable medications permitted by the Department of Health and assist with blood glucose checks.

Other Job Functions

  • Ability to proficiently read, write, speak, and understand the English language.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully and effectively with internal and external customers including residents, family members, visitors, staff, government agencies, vendors and business associates, and the general public.
  • Knowledgeable of policies and procedures, as well as DOH regulations pertaining to assisted living levels of care (ALR, EALR, SNALR).
  • Valid state of residence driver’s license and maintain a current LPN License.
  • Any other duties supervisor deems necessary.
  • Reliable transportation.

Qualifications of the Position

  • A combination of experience, education, and/or training which substantially demonstrates the following knowledge and skills:
  • Education: Experience: Two (2) years of experience as a licensed Practical Nurse in a care facility or other related health care facility.
  • Licenses / Certifications: Must possess a current, unencumbered, active license to practice as an LPN in New York State,

    Pay Range

    $29 - $35 per hour
