About the role

This is a nursing administrative support role to the Unit Manager that requires attention to detail, strong communication skills, and the ability to navigate the operations of a busy, subacute environment. The Assistant Unit Coordinator / Charge Nurse will be interacting and communicating with physicians, families, fellow staff members, vendors, and residents. This is a great opportunity for a seasoned nurse who is looking to move into an administrative support role, while keeping nursing skills in use on a consistent basis.

Responsibilities

  • Manage both communication and workflow so residents, families, and physicians obtain accurate and updated information as soon as possible
  • Interact with residents, families, the interdisciplinary team, and medical professionals in a positive and friendly manner
  • Support the operational function of the Subacute Unit Nursing Station and its charge nurse duties, including providing informational needs to all parties as the liaison between patients, families, and their PCPs
  • Assist in managing appointments, transportation, and communication flow of the short-term rehabilitation unit
  • Provide direct activities that involve coordination of the electronic health record (EHR) consisting of entering, transcribing, and processing physician's orders, ensuring that the electronic medication administration and treatment records are up to date and accurate
  • Ensure that subacute-unit programs and systems are in compliance, including: C.N.A. documentation is completed each date and assesses its accuracy, reporting to the Unit Manager any inaccuracies for correction
  • Review the clinical dashboard in the EHR daily to ensure scheduled assessments are completed timely as delegated by the Unit Manager
  • Review Hospital Transfer Log to ensure completeness and accuracy of transfer information
  • Maintain resident appointment scheduling and collaborate with the Unit Manager to ensure that only necessary appointments during the resident's stay are provided by the facility
  • Maintain center's Ambulance Log and provide it to the Administrator monthly
  • Communicate to the physician and resident's responsible person when there is a change in condition that affects resident's plan of care
  • Collaborate with and communicate Rehab and Social Services identified post-discharge appointment needs to ensure scheduling as required
  • Ensure that necessary nursing information related to change of condition and treatment plans are communicated to the team during Nursing Huddles

Qualifications

  • Graduate from an accredited school of Nursing
  • Licensed RN or LPN in Massachusetts
  • At least one (1) year working as a Licensed Practical Nurse or Registered Nurse
  • Demonstrates the ability to read, write, and speak English, as well as add, subtract, multiply and divide
  • Proficiency with computer skills including Microsoft Office and an EMR (PCC preferably)
