About the role

Our Neuro Transitional Center provides a unique, comfortable environment designed to feel like home. It gives patients with physical, behavioral and cognitive impairments access to comprehensive therapy in multiple settings. Specialized therapists help transitional patients regain their independent living skills — with the goal of safely functioning in their home and community.

Responsibilities

  • Oversees and coordinates medical rehabilitation services in alignment with the Neuro Transitional Center’s policies, procedures, and treatment model.
  • Ensures patient safety and fosters a positive patient experience throughout the continuum of care.
  • Supervises Neuro Rehabilitation Coach Supervisors and Neuro Rehabilitation Coaches, providing leadership and support to maintain high standards of care and staff performance.
  • Collaborates with the Rehab Residential Manager to manage residential operations, including both administrative and clinical responsibilities, while contributing to a safe, therapeutic, and well-organized environment.
  • Assists the Residential Manager with ensuring that the residential facility operates in compliance with HHSC ALF Licensure, CARF and other regulatory/accreditation organizations, including assisting with creating procedures to ensure adherence.
  • Manages documentation ensuring completeness, timeliness and accuracy in accordance with specialty area.
  • Communicates with On Call and/or physicians/office nurse, as needed.
  • Ensures communication of pertinent information including specific concerns regarding patient safety or health to the treatment team in a timely manner. Ensure residential communication are up to date.
  • Aids in the development and tracking of transdisciplinary goals within the patient plan of care.
  • Aids the Residential Manager with the oversight of clinical and regulatory compliance, orientation, training, supervision, and ongoing education and competency assessments of residential direct care staff across the delivery sites and in the community.
  • Participates in on-call rotation to provide support to staff after hours and on weekends.
  • Participates in required meetings which may include patient staffing, family conferences, admissions, EOC, residential meetings, etc.
  • Occasionally transports patients on community outings.
  • Trains and oversees nurse delegation.
  • Conducts weekly MAR audits, verification of medication administration records and input of orders into EHR.
  • Conducts head-to-toe assessment of new admits, and ongoing required assessments.
  • Completes residential intake and ensures all admission orders are entered into PCC.
  • Communicates with pharmacy regarding new or changed orders and ensures medications are delivered on time.
  • Provides education to patients and families regarding diagnosis and the care required as a result of their injury.

Qualifications

  • Current licensure as a Registered Nurse (RN) required.
  • BLS certification through the American Heart Association is required.
  • Current State Driver's License in good standing required.
  • Nursing Facility Administrator License as defined by state requirements, required within 90 days or as defined by regulation.
