About the role

The role involves coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, maintaining trip logs for van use, and making independent judgments to benefit the individual.

Responsibilities

  • Making independent judgments using discretion for the benefit of the individual.
  • Ensuring individuals receive the highest standard of services required and their day-to-day needs are met.
  • Ensuring all services are followed as written in the individual’s plans.
  • Assessing the needs of the whole individual/family including their basic needs such as food, clothing, and shelter.
  • Referring individuals to appropriate internal and external services as required to meet their families' needs.
  • Following up on referrals made on behalf of an individual’s family and responding to all referrals made to you/your program.
  • Participating in cross-program service reviews that support the planning and coordination of service delivery and service closure.
  • Organizing records and ensuring they are up to date.
  • Attending interdisciplinary team meetings/staff meetings.
  • Creating agendas for meetings, planning standard items, such as committee reports, announcements, and staff concerns.
  • Collaborating with consultants, especially psychologists, on programming.
  • Following through on recommendations for U.R. committees and attending U.R. meetings when delegated.
  • Completing and maintaining investigation certification as it relates to incidents within the DDS.
  • Maintaining and submitting timely and accurate records and reports regarding the health and safety of the individuals and the quality of each program.
  • Submitting, updating, and closing all incidents for the program.
  • Maintaining compliance with OPWDD and the Justice Center regarding incidents.
  • Maintaining awareness of and sensitivity to cultural and socioeconomic characteristics of persons and populations served.
  • Preparing staff schedules of Program Specialists, including responsibility for vacation, personal, and sick leaves, and maintaining coverage during any emergency periods of staff shortages.
  • Evaluating individual work performance of all staff through observations and performance counseling in conjunction with the Program Manager.
  • Ensuring each individual is being provided and/or assisted in skill development in areas such as activities of daily living, domestic living, recreation, and socialization as needed.
  • Aiding Program Manager in devising and executing relevant agency/program policies, in accordance with OPWDD and/or other funding agency guidelines.
  • Conducting frequent rounds on all shifts and submitting reports to Program Manager to ensure that safety and health requirements are observed, that the physical environment is comfortable and stimulating, and that quality individual care and programming are provided.
  • Tracking appointments, communicating with CMS, Nurse, relevant doctors, and managers, ensuring accuracy of medication inventory, recording and ordering refills when necessary, and following up on medical appointments.
  • Completing medical referral forms and conducting monthly medical book reviews.
  • Ensuring coordination by supervisors of all transportation activities to assure maximum use of transportation services for day programming, recreation services, medical, and special requirements.
  • Ensuring the transporting of individuals to and from programs, community inclusion activities, medical appointments, home visits, and all other activities as needed.
  • Ensuring the timely receipt and ongoing availability of all individual’s needs (toilet items, clothing, and personal effects).
  • Preparing Day Program progress reports at ITT reviews as assigned.
  • Distributing petty cash funds to staff or collecting petty cash receipts and submitting them to the Program Manager.
  • Conducting monthly fire evacuation drills including all staff members on all shifts.
  • Participating in admission and discharge reviews and making recommendations to Program Manager.
  • Developing monthly recreation schedules in consultation with Program Manager.
  • Implementing and monitoring in-house and community recreational activities, providing hand-on assistance as needed.
  • Directing care as necessary.
  • Attendance standards: 100% present - 900 PM, Sunday through Thursday, and at the discretion of the Program Manager.

Requirements

  • Highschool Diploma with one (1) year experience in Human Services (Social Work, Psychology, Special Ed., etc.) or related field required, Associate’s Degree preferred.
  • Two (2) years minimum experience working with individuals diagnosed with intellectual disability (ID).
  • AMAP, SCIP-R, and First Aid/CPR certified.
  • A valid driver’s license is required (must be able to drive large vehicles which consist of wheelchair or 15-seater van).
