About the role
The position involves coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, maintaining trip logs for van use, and ensuring individuals receive the highest standard of services.
Responsibilities
- Making independent judgments using discretion for the benefit of the individual.
- Ensuring individuals receive the highest standard of services required and their day-to-day needs are met.
- Ensuring all services are being followed as written in the individual’s plans.
- Traveling and having good writing, articulation, and advocacy skills.
- Assessing the needs of the whole individual/family including their basic needs such as food, clothing, and shelter.
- Making referrals to appropriate internal and external services as required to meet the individual’s families’ needs.
- Following up on referrals made on behalf of an individual’s family and responding to all referrals made to you/your program.
- Participating in cross-program service reviews that support the planning and coordination of service delivery and service closure.
- Organizing records and ensuring they are up to date.
- Attending interdisciplinary team meetings and staff meetings.
- Creating an agenda for meetings, planning standard items, and attending U.R. meetings.
- Collaborating with consultants, especially psychologists, on programming.
- Completing and maintaining investigation certification as it relates to incidents within the DDS.
- Maintaining and submitting timely and accurate records and reports regarding the health and safety of the individuals and the quality of each program.
- Ensuring all safeguards of life safety situations with individuals are implemented and followed.
- Ensuring coordination by supervisors of all transportation activities to assure maximum use of transportation services for day programming, recreation services, medical, and special requirements.
- Ensuring the transporting of individuals to and from programs, community inclusion activities, medical appointments, home visits, and all other activities as needed.
- Ensuring the timely receipt and ongoing availability of all individuals’ needs (toilet items, clothing, and personal effects).
- Preparing day program progress reports at ITT reviews as assigned.
- Distributing petty cash funds to the staff or collecting petty cash receipts and submitting them to the Program Manager.
- Conducting monthly fire evacuation drills including all staff members on all shifts.
- Participating in admission and discharge reviews and making recommendations to Program Manager.
- Developing monthly recreation schedules in consultation with Program Manager.
- Implementing and monitoring in-house and community recreational activities, providing hand-on assistance as needed.
- Directing care as necessary.
Requirements
- A high school diploma with one (1) year experience in Human Services (Social Work, Psychology, Special Ed., etc.) or related field required, an associate’s degree preferred.
- Two (2) years minimum experience working with individuals diagnosed with intellectual disability (ID).
- AMAP, SCIP-R, and First Aid/CPR certified.
- A valid driver’s license (must be able to drive a large vehicle which consists of wheelchair or 15-seater van).
