Pay
Min $75,000.00 – Max $85,000.00 per year. Compensation will be based on experience, qualifications, credentials, employment status, and work schedule.
Requirements
- Master’s Degree in Social Work or a human services field
- CSW or LSW within the state of operations
- 3+ years of Social Work Experience
- Long term care/skilled nursing experience preferred
- Practice Setting: Sub-Acute Care, Short Term Rehab, Skilled Nursing, Long Term Care
Responsibilities
- Plan and direct a comprehensive social service program that provides support services for residents and families
- Participate in an interdisciplinary team approach to serve the needs of the Center population
- Provide therapeutic intervention to help residents cope with social and psychological aspects of illness, impairments, disabilities, institutionalization, and separation from family
- Maintain written documentation in resident medical records per company policy
- Perform initial evaluation/assessment of each resident’s psychosocial needs and document at least quarterly; short-term residents require ongoing documentation
- Develop the social service component of the resident’s plan of care, identifying problems, goals, and approaches
- Participate in and coordinate interdisciplinary care planning conferences at admission and required intervals to revise the overall plan of care
- Ensure residents and/or families have input into the care planning process
- Act as a liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights
- Conduct family meetings (e.g., new admission group meetings, support groups, family and friends council)
- Perform administrative tasks such as completing necessary forms and reports under the Administrator’s direction
- Facilitate discharge planning, including developing organized discharge plans for all residents
- Ensure safety of all Center customers to minimize fire and accident risks
- Ensure the Center adheres to legal, safety, health, fire, and sanitation codes, including familiarity with disaster plans and MSDS
- Coordinate admissions by generating referrals, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours
About the Role
The Assistant Director of Social Services oversees the day-to-day coordination of the Social Service department, adhering to company policies, state and federal guidelines, and the National Association of Social Workers’ Code of Ethics. Social service functions include supervising staff, direct services, advocacy, counseling, service coordination, education, policy and program development, and community relations.
