Salary Range: $75K - $85K per year. Compensation will be based on experience, qualifications, credentials, employment status, and work schedule.
About the Role
The Assistant Director of Social Services is responsible for the day-to-day coordination and oversight of all aspects of the Social Service department. This role abides by and practices according to the company's Social Service Policies and Procedures Manual, State and Federal guidelines, and the National Association of Social Workers' "Code of Ethics."
Responsibilities
- Plan and direct a comprehensive social service program providing support services for residents and families.
- Participate in an interdisciplinary team approach to serve the needs of the Center population.
- Provide therapeutic intervention to help residents cope with social and psychological aspects of illnesses, impairments, or disabilities, as well as feelings about institutionalization and separation from family.
- Maintain written documentation in resident medical records per company policy, including initial evaluations/assessments and quarterly updates.
- Develop the social service component of residents' care plans, identifying specific problems, goals, and approaches.
- Participate in and coordinate interdisciplinary care planning conferences at admission and required intervals to assist in developing and revising care plans.
- Ensure residents and/or families have input into the care planning process.
- Act as a liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights.
- Conduct family meetings as needed (e.g., new admission group meetings, support groups, family and friends council).
- Perform administrative tasks such as completing necessary forms and reports under the direction of the Administrator.
- Facilitate discharge planning, including developing organized discharge plans for all residents.
- Ensure the safety of all Center customers to minimize potential for fire and accidents.
- Ensure the Center adheres to legal, safety, health, fire, and sanitation codes, including familiarity with fire, safety, and disaster plans, and current MSDS.
- Coordinate admissions by generating referrals, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours.
Requirements
- Master’s Degree in Social Work or a human services field.
- Possess a CSW or LSW within the state of operations.
- 3+ years of Social Work experience.
- Long-term care/skilled nursing experience preferred.
- Experience in practice settings such as Sub-Acute Care, Short-Term Rehab, Assisted Living, Skilled Nursing, LTACH, or Long-Term Care.
Pay
$75,000 – $85,000 per year.
