Assistant Director of Patient Care Services

Position Overview

The Assistant Director of Patient Care Services is the Registered Nurse (RN) responsible to assist in the overall direction of clinical services, establishing, implementing and evaluating goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organizational philosophy.

Essential Job Functions

• Receives case referrals. Reviews available patient information related to case, including home visits, to determine home health care needs. Assigns appropriate personnel to case as needed.

• Conferences with Medical Director and/or Director of Patient Care Services regarding any questions about an individual’s eligibility for services.

• Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home health services; and is available at all times during operating hours to assist clinicians as appropriate.

• Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

• Attends case conference meetings with home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

• Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and home health personnel.

• Affords assistance in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new Employees.

• Affords assistance in the planning, implementation and evaluation of inservice and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.

• Complies with accepted professional standards and principles.

• Affords assessment of health needs of community and assists in the development of plan(s)/program(s) to meet needs. Participates in public relation and community activities that promote the organization’s role as an effective member of the home health and healthcare delivery system.

• Promotes customer service orientation to all home health personnel.

• Participates actively in quality assessment performance improvement program.

• Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.

Basic Qualifications

• Six years or more (6+) in Home Health at the Supervisor level

• Current unencumbered Registered Nurse (RN) license in location state

• A graduate in Nursing from an accredited program by the National League for Nursing, AND

• Three or more (3+) years of Registered Nursing experience

• Two or more (2+) of the last five (5) years in Registered Nursing Home Health organization

• One or more (1+) years of supervisory/management experience in home health care organization

• Current CPR card

• Demonstrated ability to supervise and direct professional administrative personnel

• Knowledge and expertise in clinical decision-making and Quality Management review and Evaluation

• Ability to market and deal tactfully with customers and the community

• Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order

• Has excellent observation, verbal and written communication skills

• Knowledge of business management, governmental regulations and Joint Commission standards

Preferred Qualifications

• Bachelor’s degree in Nursing (BSN) or above from an accredited program by the National League for Nursing

• Two or more (2+) of the last five (5) years in Registered Nursing home health

Physical Requirements

• Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary.

• The employee must be able to lift and/or move up to 10 lbs. on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.

• Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
