Position Summary

The Assistant Director of Nursing assists the Director of Nursing in the overall leadership, supervision, and coordination of clinical services throughout the agency. The ADON shares responsibility for maintaining compliance with California Home Health regulations, agency policies, and professional nursing standards while supporting daily clinical operations, quality improvement, staff development, and patient care coordination. This role serves as a key member of the leadership team and helps ensure patients receive safe, compassionate, and high-quality care while supporting clinicians in delivering exceptional service.

Primary Responsibilities

  • Assist the Director of Nursing in overseeing the delivery of high-quality home health and private duty nursing services.
  • Provide clinical leadership and support to RNs, LVNs, CNAs, Home Health Aides, and caregivers.
  • Review patient assessments, plans of care, and clinical documentation for accuracy and compliance.
  • Ensure patient care is delivered in accordance with physician orders, agency policies, and professional nursing standards.
  • Evaluate patients for appropriate levels of care and service eligibility.
  • Promote patient safety, clinical excellence, and positive patient outcomes.

Compliance & Quality Assurance

  • Assist with maintaining compliance with California Home Health regulations, Medicare Conditions of Participation, HIPAA, OSHA, and agency policies.
  • Support agency readiness for State and Federal surveys, audits, and accreditation reviews.
  • Conduct chart audits, documentation reviews, and quality assurance activities.
  • Affiliate with investigating incidents, complaints, and clinical concerns and implementing corrective action plans.
  • Promote continuous quality improvement and regulatory compliance throughout the agency.

Staff Leadership & Development

  • Affiliate with recruiting, interviewing, onboarding, and orienting clinical staff.
  • Train, mentor, and coach clinicians in both office and field settings.
  • Conduct competency assessments and ongoing clinical education.
  • Participate in employee evaluations, coaching, and performance improvement initiatives.
  • Foster a collaborative, accountable, and supportive work environment.

Operations & Care Coordination

  • Collaborate with intake, scheduling, case management, and administrative teams to ensure continuity of care.
  • Affiliate with staff assignments based on patient needs, clinician competency, and availability.
  • Help resolve staffing challenges and operational issues.
  • Cook with billing and payroll departments to ensure timely and accurate clinical documentation.
  • Support day-to-day clinical operations and workflow efficiency.

Patient & Family Support

  • Serve as a clinical resource for patients, families, and staff.
  • Respond to patient concerns and service issues promptly and professionally.
  • Promote exceptional communication and concierge-level customer service.
  • Ensure patient rights, privacy, and confidentiality are maintained at all times.

Leadership Support

  • Participate in scheduled leadership on-call and escalation rotations.
  • Affiliate with urgent clinical, staffing, and operational matters after business hours as needed.
  • Support the Director of Nursing in achieving agency goals, maintaining survey readiness, and driving operational excellence.

Requirements

  • Required: Active, unrestricted California Registered Nurse (RN) License.
  • Required: Associate Degree in Nursing or higher.
  • Required: Current CPR/BLS certification.
  • Required: Valid driver’s license and reliable transportation.
  • Required: Minimum three years of Home Health experience.
  • Required: Minimum two years of supervisory or leadership experience in a healthcare setting.
  • Required: Strong knowledge of Home Health regulations, clinical documentation, and quality assurance.
  • Required: Excellent leadership, communication, organizational, and problem-solving skills.
  • Required: Ability to travel to patient homes and agency locations as needed.
  • Preferred: Leadership experience within Home Health or Private Duty Nursing.
  • Preferred: Bachelor of Science in Nursing (BSN).
  • Preferred: Experience preparing for or participating in regulatory surveys and audits.
  • Preferred: Familiarity with AlayaCare, CareAcademy, or similar electronic medical record systems.
  • Preferred: Experience in concierge healthcare or high-touch patient care environments.
