Position Summary

Responsible for assisting the Director of Nursing with day-to-day clinical and administrative duties as well as ensuring that the residents' proper care and oversight is being conducted as outlined in community policies and procedures.

Essential Duties And Responsibilities

  • Maintains communications with Healthcare Providers regarding recent changes or requests for orders.
  • Ensures that the community follows all federal, state, and local laws and regulations pertaining to clinical care.
  • Collects and reports occurrences, and if necessary, assists in developing/implementing improvement plans.
  • Partners with the Administrator and other team members to maintain resident and team member health safety.
  • Partners with the pharmacy consultant to provide optimal pharmaceutical services to residents.
  • Responsible for infection control practices in the community.
  • Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when the floor nurse is not available.
  • Identifies ongoing needs and services of residents through the evaluation/Service Plan process including documentation for residents with a change in health care status.
  • Completes any assigned evaluations.
  • PARTNERS WITH OTHER DEPARTMENT COORDINATORS TO IDENTIFY, REVIEW, AND DISCUSS POTENTIAL CHANGES IN SERVICE/MEDICATION LEVELS AND UPDATES THE RESIDENT’S SERVICE PLAN AS INDICATED BY STATE REGULATIONS.
  • Maintains communications with resident's family and/or responsible party regarding changes in care or health concerns.
  • Other duties as assigned.

Supervisory Responsibilities

Provides supervision to Certified Nursing Assistants and all direct care staff as assigned to assure delivery of consistently high quality care to residents.

CARRIES OUT SUPERVISIONAL RESPONSIBILITIES IN ACCORDANCE WITH THE ORGANIZATION'S POLICIES AND APPLICABLE LAWS.

Educational Requirements And Experience

  • Maintains current state license as a Licensed Practical Nurse per state regulations.
  • Experience in assisted living, home health, or long-term care industries.
  • TWO (2) YEARS EXPERIENCE AS A LPN.
  • MUST DEMONSTRATE COMPETENCE IN ASSESSMENT SKILLS, INJECTIONS/MEDICATION ADMINISTRATION, FOLLOW UP, CHARTING, TRIAGE, COMMUNICATION, AND EFFECTIVE TEAM LEADING/SUPERVISION.
