About the role
CareCore at Mentor is seeking an experienced and motivated Registered Nurse (RN) to join our leadership team as our Assistant Director of Nursing (ADON) / Infection Preventionist. This is a great opportunity for an RN with strong long-term care experience who enjoys being hands-on, supporting clinical staff, and maintaining high standards for infection prevention, resident care, and regulatory compliance.
Responsibilities
- Assist the Director of Nursing with day-to-day clinical operations
- Serve as the facility’s Infection Preventionist
- Monitor, track, and investigate infections and infection trends
- Maintain infection prevention and control policies, procedures, and documentation
- Provide infection control education and training to staff
- Assist with survey readiness and regulatory compliance
- Support nursing staff through education, coaching, and clinical oversight
- Participate in audits, quality improvement initiatives, and resident care reviews
- Assist with staffing, employee performance, and clinical concerns as needed
- Provide leadership coverage and support to the DON and nursing department
Qualifications
- Active Ohio RN license required
- Previous long-term care/skilled nursing experience required
- Infection prevention/control experience strongly preferred
- Previous nursing leadership or supervisory experience preferred
- Strong knowledge of state and federal LTC regulations
- PCC/PointClickCare experience preferred
- Strong communication, organization, and leadership skills
- Must be comfortable working collaboratively and providing hands-on support when needed
