Essential Functions
- Works directly with the residential or day habilitation staff under the supervision and direction of the Behavior Intervention Specialist (BIS), Manager and Coordinator.
- Works with the team to look at each person supported in a wholistic manner, identifying and meeting their strengths, preferences and needs.
- Identifies behavioral, psychiatric, social, vocational, health, nutritional and mobility needs of each person and brings these to the attention of the team.
- Aids in conducting assessments and observations, developing goals/outcomes, and ensuring accurate and consistent implementation of these goals.
- Develops effective data collection systems to monitor and measure progress, and makes revisions as needed.
- Supports the program in ensuring all programming is implemented consistently and according to agency policy.
- Aids in monitoring the completion of habilitation-related paperwork and sharing reports with appropriate service providers.
- Aids in ensuring interventions receive proper approval before implementation (including informed consent and human rights).
- Develops and implements group and individual programming, and assists with staff training.
- Facilitates case conferences and functions as a member of the interdisciplinary team, collecting and synthesizing reports into CFA or case conference minutes.
- Develops and trains staff on goals/outcomes, supports the team in creating a staff action plan, and participates in weekly IDT meetings.
- Attends team meetings held on behalf of the people supported by other service providers, and supports the team in working with families, members of the community, and other team meetings.
- Maintains accepted standards of confidentiality and provides clinical information to staff working directly with individuals.
- Assists the IDT in initiating appropriate referrals for necessary consultations and evaluations, and ensures individuals receive appropriate mental and physical health care.
- Facilitates coordination of individual treatment between the residence and the day hab, and visits individuals at their day/vocational program as needed.
- Maintains habilitation records in accordance with OPWDD and BIRCH policy under the direction of the IDT, QA staff, Psychology Coordinator, and Nurse Administrator.
- Brings programmatic and individual clinical issues to the attention of the IDT and resolves them in a solution-oriented manner.
- Meets with supervisors regularly, including the Residential Manager/Coordinator, BIS, and RN, prepared with an agenda and following up on tasks delegated during supervision.
- Functions in a direct service professional capacity, working under the direction of the Residential Manager, Operations Coordinator, BIS, and RN.
Qualifications
- Minimum of 3 years’ experience working in OPWDD related field and preferably experience in a residential, health or treatment related setting preferred.
- Demonstrated competency in written, verbal, and computational skills to document records in accordance with program standards.
- Ability to work a flexible schedule if needed to accommodate working with the individuals in the residence and attending team meetings.
