Essential Functions
- Works directly with the residential or day habilitation staff under the supervision and direction of the Behavior Intervention Specialist (BIS), Manager and Coordinator.
- Works with the team to look at each person supported in a wholistic manner, identifying and meeting their strengths, preferences and needs.
- Identifies behavioral, psychiatric, social, vocational, health, nutritional and mobility needs of each person and brings these to the attention of the team.
- Aids in conducting assessments and observations, developing goals/outcomes, and ensuring accurate and consistent implementation of progress reports.
- Supports the program in ensuring all programming is implemented consistently and in accordance with agency policy.
- Aids in monitoring and sharing habilitation related paperwork with appropriate service providers.
- Aids in ensuring interventions receive proper approval prior to implementation, if required.
- Develops and implements group and individual programming, assists with staff training, facilitates case conferences, and functions as a member of the interdisciplinary team.
- Supports the team when working with families, members of the community, and attends team meetings.
- Provides clinical information to staff and maintains confidentiality.
- Assists the IDT in initiating appropriate referrals and ensuring individuals receive appropriate mental and physical health care.
- Facilitates coordination of individual treatment between the residence and the day hab.
- Visits individuals at their day/vocational program and attends case conferences and other special IDT meetings as needed.
- Maintains habilitation records in accordance with OPWDD and BIRCH policy under the direction of the IDT, QA staff, Psychology Coordinator, and Nurse Administrator.
- Brings programmatic and individual clinical issues to the attention of the IDT and resolves them in a solution-oriented manner.
- Meets regularly with supervisors including the Residential Manager/Coordinator, BIS, and RN.
Qualifications
- Minimum of 3 years’ experience working in OPWDD related field and preferably experience in a residential, health or treatment related setting preferred.
- Demonstrated competency in written, verbal and computational skills to document records in accordance with program standards.
- Ability to work a flexible schedule if needed to accommodate working with the individuals in the residence and to attend team meetings.
