Responsibilities

  • Conduct diagnostic and risk assessments using evidence-based tools
  • Determine appropriate level of care and support safe, timely patient transitions
  • Provide crisis intervention and safety planning

  • Document assessments and care plans thoroughly in the EHR
  • Coordinate with community resources to support patient needs
