AmeriCorps Member – Smoking Cessation Coordinator

About the Role

Healthy Communities is CHC’s AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care, and support. This program also identifies the unique needs of communities and neighborhoods and establishes strategic plans to address them. Service delivery directly addresses the public health crisis in vulnerable communities, making healthcare and support services accessible through flexible delivery.

This is an AmeriCorps Member role – Smoking Cessation Coordinator – serving within the Center for Key Populations’ Substance Use Health Program, based in Middletown but may support activities at other CHC sites.

Terms & Benefits

• One-year service term from September 8, 2026, through August 31, 2027
• Serve a minimum of 33 hours a week for a total of 1,700 hours during the service year
• $25,000 stipend over the course of one year, paid bi-weekly
• An education award of $7,395 at the end of successful service term completion

This is not a staff or volunteer position.

Responsibilities

• Conduct monthly patient health education workshops; monitor the completion of pre-post surveys by workshop participants; and complete data entry for workshops and surveys.
• Track and document all completed patient engagement activities in the patient’s electronic health record.
• Work with CHC leadership to choose and implement a smoking cessation intervention.
• Develop a curriculum for staff and patients around the chosen intervention.
• Implement and facilitate trainings for staff and education sessions for patients.
• Establish a Smoking Cessation Promotion Plan for CHCI.
• Work with Regional VP’s and Deputy VP’s to implement outreach and education on smoking cessation resources.
• Work with site supervisors to implement smoking cessation programs and conduct outreach to patients.
• Aid in the development and dissemination of Substance Use Health (SUH) educational materials and pamphlets that are inclusive of all types of substance use.
• Plan and implement community events that promote awareness and education on varying types of substance use, including tobacco, vaping, and alcohol use, along with treatment options available.
• Work with CHCI leadership to establish a smoking cessation database for tracking and monitoring of referrals.
• Work with the QI team to address smoking cessation performance improvement goals.
• Conduct Social Determinants of Health (SDOH) screenings for patients (new to care and existing high-risk) to ensure appropriate linkages to care and resources.
• Develop a statewide community resource list that provides enhanced resources for patients in high-risk categories.
• Plan and implement community events that promote all SUH programs.
• Create presentations and educational pamphlets for the SUH program.
• Conduct trainings with staff, community providers, and patients.

Qualifications

• High school diploma or equivalent required
• Proficiency in Microsoft Office and internet-related applications
• Excellent time management and organizational skills
• Excellent oral and written communication skills
• Sound judgment and decision-making skills
• Demonstrated ability to problem-solve and remain calm during a crisis
• Successful clearance of all required criminal history checks (NSCHC)
• Able to travel between CHC sites and in-state
• Ability to serve in the evenings or weekends, occasionally, due to community events and educational sessions

Preferred Qualifications

• Associate’s degree in public health, social work, or a related field
• Bilingual (English/Spanish)
• Understanding of and ability to access community resources
• Ability to work collaboratively, building relationships and teams
• Experience in providing crisis intervention, coordination of services, advocacy, and community outreach in working with vulnerable populations
