Ambulatory Urology-Urologic Specialty Nurse Navigator
Responsibilities
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
Implements, coordinates, manages and evaluates a comprehensive urologic outpatient practice. This includes, but is not limited to, overseeing outpatient care plans, coordinating outpatient clinics, triaging patient calls and communicating with referring providers to ensure a safe and effective transition from referral through the postoperative period.
Works collaboratively with members of the division to ensure the delivery of safe quality driven care. Supports the divisional efforts to ensure provision of safe and efficient care for this target patient population. Identifies initiatives, collects, analyzes, tracks and trends data to ensure compliance with divisional, departmental, and external quality initiatives.
Requirements
- Registered Nurse (RN), BSN required
- 3-5 years of nursing experience required
- Prior experience with urologic patients preferred
Qualifications
- Must have a valid New York State RN license
- Must be able to pass a background check
