Nashville General Hospital is seeking a compassionate and resourceful Ambulatory Navigator to join our ambulatory primary care and specialty clinic. The ideal candidate will assist patients in navigating the healthcare system, ensuring coordinated, accessible, and patient-centered care within a Patient-Centered Medical Home (PCMH) model.

Responsibilities

  • Care coordination for physician patients, including interviewing and directing visiting personnel and patients to appropriate offices.
  • Answer telephones, schedule appointments, and perform related receptionist duties.
  • Maintain records of appointments and meetings for physicians as needed.
  • Ensure high-level customer service in the clinic and resolve issues promptly.
  • Provide suggestions for service improvement.
  • Enter patient data into designated registry/documentation system and generate reports as needed.
  • Coordinate scheduling of patient visits and referrals.
  • Actively support and participate in community events supported by physicians.
  • Provide follow-up during transitions, including telephonic follow-up or home visits to ensure smooth transitions from hospital/community facility to home.
  • Ensure consistent implementation of the treatment plan with the Care Coordination Team (CCT) through ongoing communication with healthcare team members.
  • Collaboratively function across the continuum of care.
  • Utilize health behavior change techniques to identify patient readiness for self-management and barriers to optimal care; implement interventions to address patient needs.
  • Document care into electronic health record or registry.
  • Participate in hospital committees and initiatives related to improving operations, patient education, gap closure, and reducing preventable hospital readmissions.
  • Actively support initiatives in Payer Pay for Performance or Quality Improvement Plans as designated by the CCT Director.
  • Effectively communicate with physicians to ensure an effective continuum of care, including daily communication, financial interventions/recommendations, and assisting in recognizing and resolving care gaps.
  • Scribe for physicians as needed to optimize patient flow.
  • Acquire knowledge of accountable care act requirements to maintain compliance and improve value-based proposals and pay-for-performance measures.
  • Work with Marketing on physician education programs and community education and marketing.
  • Prepare statistical data, financial statements, and reports for granting entities, as well as correspondence and administrative reports.

Requirements

  • Experience in care coordination for individuals with chronic conditions and complex medical and social needs.
  • Experience participating in hospital care transition improvement programs.
  • Ability to adapt to evolving goals and requirements and test new approaches to improve patient care.
  • Excellent leadership, verbal, written, and interpersonal skills.
  • Critical thinking and clinical problem-solving skills.
  • Proficiency in Electronic Health Records, Microsoft Products (Word, Excel, Outlook, Access, PowerPoint), and experience with database and patient registries.
  • Five years of experience in the healthcare field, navigating the exchange, and/or financial counseling experience (preferred).

Qualifications

  • Two (2) years of college education or equivalent experience (required).
  • Bachelor’s degree (preferred).
