Ambulatory Clinical Pharmacist Specialist - Neurology, FT

About the Role

Provides patient care through direct operational and/or clinical patient services. Collaborates within the department to enhance operations and clinical programs. Participates in departmental and/or institutional committees, education, and promotion of the profession. This position requires working weekends, evenings, and holidays as needed. Acts as a mentor to pharmacy students and/or residents.

The Clinical Pharmacist Specialist collaborates with the multidisciplinary team to improve care, achieve optimal outcomes, and provide continuity of care through pharmacy patient care services, including therapy care plan development, medication management, education, monitoring, and follow-up. Provides evidence-based and cost-effective comprehensive medication management for patients with chronic conditions. Delegated authority to obtain patient medical history, order and evaluate laboratory results, and make medication decisions, including modifying or discontinuing treatment within scope of practice.

Responsibilities

• Provides accurate medication order-entry, drug distribution, and patient care.
• Confirms the appropriateness of drug therapy for neonates, pediatrics, adolescents, adults, and geriatrics.
• Minimizes turnaround time for products and services.
• Actively participates and leads department and institutional committees and projects (e.g., performance improvement, formulary management, regulatory, patient safety, financial).
• Provides concise, applicable, and timely responses for drug information requests and other pharmaceutical care services through an on-call service, with documentation in the patient care record when appropriate.
• Represents Prisma Health within professional organizations through service, presentations, and/or publications.
• Provides experiential education, evaluation, and training for technicians, students, pharmacy residents, medical students/residents, and new employees.
• Provides leadership and pharmaceutical excellence through direct patient care activities as a member of a multidisciplinary medical team.
• Shares clinical and scholarly activity to promote evidence-based medication use and advance the pharmacy profession.
• Provides lab review, clinical services, patient education, and corresponding documentation via departmental clinical initiatives.
• Performs other duties as assigned.

Requirements

• Bachelor’s degree from an ACPE-approved college of Pharmacy; PharmD preferred.
• Two (2) pharmacy residencies or ten (10) years of specialty pharmacy experience. Alternatively, completion of 1 pharmacy residency with 4 years of specialty pharmacy experience.
• In lieu of an active Pharmacist license, may be a graduate of an ACPE-accredited U.S. college of Pharmacy with an intern certificate pending initial licensure or have completed requirements for reciprocity pending board of pharmacy interview for licensure.

Qualifications

• Licensed to practice as a pharmacist or eligible to become licensed within 4 months of hire date in the state of employment.
• Board Certification in Pharmacy. In the ambulatory setting, BCACP is accepted in lieu of BCPS.
• Knowledge of medical and pharmacy terminology, mathematical, computer, and aseptic technique skills.
• Knowledge of State BOP/Dept of Health/DEA rules and regulations.
• Ability to handle urgent and stressful working conditions.
