Ambulatory Clinical Pharmacist - Neurology

About the Role

This position assumes responsibility for the medication management of assigned patients. Responsibilities include monitoring and evaluating medication therapy for appropriateness based on the patient's health status, institutional criteria/guidelines, and cost-efficient measures. This involves medication reconciliation, medication monitoring, and patient education. The role also includes educating Pharmacy and Medical Residents, providing education to healthcare providers as needed, and contributing pharmacy expertise to order sets, benefit design, formularies, and other pharmacy-related documents.

Responsibilities

• Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug selection and dosing, and communicates findings to healthcare practitioners.
• Evaluates patient response to drug therapy, documents interventions, and conducts outcome studies/evaluations to ensure good patient outcomes.
• Assists with the development of pathways, best practices, and guidelines for assigned patient populations.
• Educates staff, students, patients, healthcare providers, and other professionals; submits for publication once every two years.
• Mentors residents, students, and new staff to meet departmental goals.
• Ensures overall patient care beyond the hospital visit, through all transitions of care.
• Accommodates special organizational needs by volunteering for tasks during staffing shortages caused by inclement weather, power outages, computer downtime, or other situations.
• Adapts behavior to specific patient populations, including respect for privacy, communication style, and method of introduction.
• Remains knowledgeable on current federal, state, and local laws, accreditation standards, or regulatory requirements applicable to the assigned area and ensures compliance.
• Performs other related duties as required.

Requirements

• Bachelor of Science in Pharmacy (required).
• Completion of a PGY1 Pharmacy Practice Residency (required; 3 years of related specialty experience accepted in lieu of PGY1 Residency).
• Doctor of Pharmacy (PharmD) (preferred).
• Completion of a PGY2 Pharmacy Residency (preferred).
• Current Pharmacy license in the state of practice (required).

Skills

• Computer skills and dexterity required for data entry and retrieval of patient information.
• Proficiency with Windows-style applications and keyboard.
• Effective verbal and written communication skills; ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
• Strong leadership and teaching skills.

Physical and Environmental Demands

• Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
• Must be able to stand for prolonged periods of time.
• Must be able to travel throughout and between facilities.
• Duties performed routinely require exposure to blood, body fluid, and tissue.
• Works with patients who have known or suspected communicable diseases and may enter isolation rooms.
• Occupational risk for exposure to all communicable diseases, hazardous medications, and hazardous waste.
• Potential exposure to hazardous medications or waste through receipt, transport, storage, preparation, dispensing, administration, cleaning, and/or disposal of contaminated waste.
