Ambulatory Clinical Pharmacist - Primary Care Clinical Pharmacy Services

About the role

This position monitors and evaluates drug therapy for appropriateness based on the patient’s health status, institutional criteria/guidelines, and cost-efficient measures. You will assume responsibility for the management of pharmacy services in assigned areas with the goal of improving patient outcomes. The role includes providing drug information to healthcare personnel, participating in patient care rounds, precepting pharmacy students and residents, and delivering in-services/education to pharmacy and other healthcare providers. Additionally, this position will handle refill requests for Slidell Primary Care to expedite the process, save providers' time, and promote patient safety.

Responsibilities

• Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug selection and dosing, and communicates findings to healthcare practitioners.
• Evaluates patient response to drug therapy and documents interventions to ensure good patient outcomes.
• Assists with the development of pathways, best practices, and guidelines for assigned patient populations.
• Educates staff, students, patients, healthcare providers, and other professionals; submits work for publication at least once every two years.
• Mentors residents, students, and new staff to meet departmental goals.
• Responsible for overall patient care beyond the hospital visit, through all transitions of care.
• Accommodates special organizational needs by volunteering for additional tasks during staffing shortages caused by inclement weather, power outages, computer downtime, or other situations.
• Adapts behavior to the specific patient population, including respect for privacy, communication style, and method of introduction.

Requirements

• Completion of a PGY-1 Pharmacy Practice Residency (3 years of related specialty experience can be accepted in lieu of PGY-1 Residency).
• License to practice Pharmacy by the Louisiana Board of Pharmacy.
• Computer skills and dexterity required for data entry and retrieval of patient information; proficiency with Windows-style applications and keyboard.
• Effective verbal and written communication skills, with the ability to present information clearly and professionally.
• Strong leadership and teaching skills.

Preferred Qualifications

• Doctor of Pharmacy (PharmD).
• Completion of a PGY-2 Specialty Pharmacy Residency.
• Ambulatory experience.

Physical and Environmental Demands

The physical essential functions of this job include frequently exerting 10 to 20 pounds of force to move objects and occasionally exerting up to 100 pounds of force.
