Clinical Pharmacist- Ambulatory Care

About the Role

The University of Iowa Health Care, Department of Pharmaceutical Care, is seeking an experienced and dedicated Clinical Pharmacist to provide expert care for patients in our Ambulatory Care Pharmacies. This role provides pharmaceutical care, including clinical support, patient education, medication dispensing, and ongoing monitoring for patients receiving medications through ambulatory pharmacies, including immunization services. The Ambulatory Care Clinical Pharmacist also supports ambulatory care operations at a central fill pharmacy location, serves as a role model for staff, residents, and students, and contributes to the growth of ambulatory pharmacy services. Selective candidates are eligible for a sign-on bonus up to $10,000.

Responsibilities

• Provides Therapeutic Drug Monitoring (TDM) on clinic-administered medications and take-home prescription orders.
• Completes comprehensive patient assessments, including evaluation of medications and medical history.
• Evaluates prescription orders for appropriateness of indication, dose, route, interactions, adjustments for organ dysfunction, incompatibilities, side effects, cost-effectiveness, duplicate therapy, and potential misuse or abuse.
• Ensures pharmacokinetic assessment of drug dosing through clinical evaluation, preparation of dosing recommendations, and communication with prescribers.
• Reviews and updates patient profiles based on findings during visits.
• Provides drug information to healthcare professionals and assesses drug allergies noted in medical records.
• Collects and documents medication histories and evaluates patient compliance with prescribed therapy.
• Coordinates resolution of financial concerns with department financial counselors.
• Documents all patient care activities in medical records and online reporting systems, including:
  • Patient drug allergies and specific height/weight information.
  • Medication histories and counseling.
  • Interventions with prescribers and other providers.
  • Patient interactions (in-clinic, at home, or inpatient).
  • Drug dosing recommendations and Drug Use Evaluation Program activities.
  • Steps to identify, evaluate, or avoid adverse drug reactions.
• Educates patients on pharmaceutical care services, including:
  • Medication history and allergy reviews.
  • Patient teaching classes (where appropriate).
  • Medication synchronization and adherence packaging.
  • Medication counseling and communication with local healthcare providers.
• Facilitates operational initiatives to enhance patient care, including:
  • Advocating for patients regarding drug therapy concerns.
  • Influencing therapeutic decisions to improve patient care.
  • Participating in hospital-wide programs (e.g., Drug Use Evaluation and Adverse Drug Reaction Reporting).
  • Addressing medication system problems (e.g., unclear orders, cost-effective therapy, non-formulary products).
  • Carrying out Medication Use Evaluation Program initiatives through prescriber intervention and education.
  • Serving on intra- and interdepartmental committees, task forces, and teams.
• Provides educational services, including:
  • Serving as a preceptor for pharmacy students and residents.
  • Presenting in-service educational programs to nurses, physicians, and other healthcare team members.
  • Mentoring and role-modeling for other pharmacist practitioners, particularly in ambulatory care.
• Facilitates transitions of care by participating in patient transfers from inpatient to outpatient settings, including bedside delivery of discharge medications and education.
• Completes and maintains compliance with all competencies and educational requirements.

Requirements

• PharmD (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy.
• Current licensure by the Iowa Board of Pharmacy Examiners (required upon hire).
• Immunization Certification.
• Basic Life Support Certification.
• Excellent written and verbal communication skills.
• Job-related experience with and/or commitment to service excellence.
• Professional experience working effectively with individuals from diverse backgrounds and perspectives.

Qualifications

• One (1) year of applicable pharmacy experience is preferred (prior work or student rotational experience may be considered).
• Completion of an ASHP-accredited PGY1 Residency.
• Health system pharmacy experience.
• Specialty Pharmacy experience.
• Non-sterile compounding experience.
• Board certification in Ambulatory Care, Pharmacotherapy, Oncology, Specialty Pharmacy, or other pertinent areas.
