Ambulatory Clinic RN Care Coordinator - Radiation Oncology

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
• Develop expected patient outcomes, define appropriate resource utilization, and work with the provider to set patient goals.
• Provide nursing support/coordination to the practice to optimize patient outcomes and improve patient’s quality of life through clinical and educational programs and/or support services.
• Support patient self-management of disease.
• Care for high-risk patients, including those with multiple comorbidities or high risk for readmission to a hospital setting.
• Develop relationships with patients and assess their health care needs, safety issues, and benefits.
• Educate patients and appropriate ancillary personnel about health care needs, safety issues, and benefits, including patient education.
• Utilize available electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
• Observe, record, and report patient's condition and reactions to drugs and treatments to physicians.
• Respond to/refers incoming patient treatment-related phone calls and instructs patient and family regarding medications and treatment instructions.
• Maintain/review patient records, charts, and other pertinent information, and post test and examination results.

Requirements

• High School Graduate or GED.
• Bachelor Degree Nursing (BSN) required within 5 years for new hires.
• 2 or more years of RN experience.
• Previous medical office experience preferred.

• Strong organizational and time management skills.
• Proficiency in electronic medical records (EMR).
• Ability to manage high-risk patients and coordinate care across departments.
• Excellent interpersonal and problem-solving skills.

Qualifications

• Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing.
• Current active multistate state Register Nurse (RN) license in good standing in the state where you live.
• Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) certification.
