Ambulatory Clinic Nurse Care Coordinator (RN)-PRN Cardiology

Responsibilities

  • Manage patient flow, triage, and direct patient care.
  • Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
  • Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
  • Provide nursing support/coordination to the practice in order to optimize patient outcomes and to improve patient’s quality of life through clinical and educational programs and/or support services.
  • Support patient self-management of disease.
  • Care for high-risk patients, including those with multiple co-morbidities or high risk for readmission to a hospital setting.
  • Develop relationships with patients and competently assess their health care needs, safety issues, and benefits.
  • Educate patients and appropriate ancillary personnel about health care needs, safety issues, and benefits.
  • Utilize available electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
  • Observe, record, and report patient's condition and reactions to drugs and treatments to physicians.
  • Respond to/refers incoming patient treatment-related phone calls.
  • Instruct patients and families regarding medications and treatment instructions.

Requirements

  • High School Graduate
  • Bachelor Degree Nursing (For existing employees in role, BSN required within 5 years)
  • 2 or more years RN experience.
  • Preferred: TUKHS Ambulatory Clinic LPN with 4 years of TUKHS experience that would remain in the same clinic setting.
  • Previous medical office experience.

Skills

  • Must be able to perform the professional, clinical, and/or technical competencies of the assigned unit or department.

Qualifications

  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing.
  • Current active multistate state Registered Nurse (RN) license in good standing in state in which you live.
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA).
