Ambulatory Clinic Nurse Care Coordinator (RN)-Cardiology

Responsibilities

• Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
• Provide nursing support/coordination to the practice to optimize patient outcomes and to improve patient’s quality of life through clinical and educational programs and/or support services.
• Support patient self-management of disease.
• Care for high-risk patients, including those with multiple co-morbidities or high risk for readmission to a hospital setting.
• Develop relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental health care needs of the patient.
• Educate the patient and appropriate ancillary personnel about the health care needs, safety issues, and benefits.
• Maintain/review patient records, charts, and other pertinent information. Document assessments, clinical findings, plans of treatment, and progress toward goals using EMR.
• Respond to/refers incoming patient treatment-related phone calls. Instruct patients and families regarding medications and treatment instructions.
• Maintain patient confidentiality.

Requirements

• High School Graduate
• Bachelor Degree Nursing
• 2 or more years of RN experience

Skills

• Basic typing and word processing ability
• Excellent analytical, critical thinking, and problem-solving skills

Education and Certifications

• Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing - in state in which you live OR Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing - active with multistate in progress with proof of application.
• Must have current active multi-state Registered Nurse (RN) license in good standing in state in which you live OR active single state RN license in good standing in the state you will be working, with proof of application of a multi-state RN license in the state in which you live.
• Basic Life Support Heartsaver (BLS HS) (Non-Clinical) - American Heart Association (AHA) or obtained within 14 days
