Ambulatory Clinic Nurse Care Coordinator (RN)- Cardiology

This is a PRN (as-needed) position located at Indian Creek Medical Plaza 3.

About the Role

The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who provides ethical, evidence-based, safe, and compassionate nursing care. This role focuses on patient safety and quality of nursing care by identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator supports assigned physicians by communicating with patients, families, and caregivers, and relaying information to the physician. They provide support during clinic sessions and ensure patients and families understand the plan of care. The role includes verifying that all orders placed during clinic sessions are completed and that the physician is notified of results.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Serve as a liaison for communication between front desk staff, nursing staff, physicians, and clinic manager.
• Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
• Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs and support services.
• Support patient self-management of disease.
• Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
• Manage high-risk patient care, including patients with multiple co-morbidities or high risk for hospital readmission, including maintaining a registry.
• Develop relationships with patients and use assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
• Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits, including patient education.
• Utilize electronic tools, such as EMR, to document assessments, clinical findings, treatment plans, and progress toward goals.
• Observe, record, and report patient conditions and reactions to drugs and treatments to physicians.
• Educate patients and families about diagnostic procedures, medications, nutrition, and health and wellness maintenance.
• Maintain and review patient records, charts, and other pertinent information; record tests and examination results and follow up with patients.
• Post tests and examination results.
• Respond to and refer incoming patient treatment-related phone calls; instruct patients and families regarding medications and treatment instructions.
• Maintain patient confidentiality.
• Document patient care using EMR to maintain and develop patient records in compliance with practice standards.
• Perform the professional, clinical, and technical competencies of the assigned unit or department.

Requirements

• High School Graduate.
• Bachelor's Degree in Nursing (BSN required within 5 years for existing employees in the role).
• 2 or more years of RN experience.

Preferred Qualifications

• In lieu of RN experience, will consider a TUKHS Ambulatory Clinic LPN with 4 years of TUKHS experience remaining in the same clinic setting.
• Previous medical office experience.

Licensure and Certification

• Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing. Must have a current active multistate RN license in good standing in the state where you live. Staff hired prior to 2/1/2024 are required to obtain a multistate RN license by 4/30/2026.
• Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA).
