Responsibilities

  • Manage patient flow, triage, and direct patient care.
  • Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
  • Support patient self-management of disease.
  • Care coordination to optimize patient outcomes and improve quality of life.
  • Coordinate continuity of care with patients and families following hospital admission, discharge, and ER visits.
  • Manage high-risk patient care, including those with multiple comorbidities or high risk for readmission.
  • Develop relationships with patients and assess their health care needs.
  • Educate patients and appropriate ancillary personnel about health care needs, safety issues, and benefits.
  • Utilize electronic tools like EMR to efficiently document assessments, clinical findings, and treatment plans.
  • Observe, record, and report patient conditions and reactions to treatments to physicians.
  • Document assessments, clinical findings, and treatment plans using EMR.
  • Respond to/refers incoming patient treatment-related phone calls.
  • Instruct patients and families regarding medications and treatment instructions.
  • Maintain patient confidentiality.

Requirements

  • High School Graduate
  • Bachelor Degree Nursing
  • 2 or more years of RN experience
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
