Responsibilities
- Manage patient flow, triage, and direct patient care.
- Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
- Support patient self-management of disease.
- Care coordination to optimize patient outcomes and improve quality of life.
- Coordinate continuity of care with patients and families following hospital admission, discharge, and ER visits.
- Manage high-risk patient care, including those with multiple comorbidities or high risk for readmission.
- Develop relationships with patients and assess their health care needs.
- Educate patients and appropriate ancillary personnel about health care needs, safety issues, and benefits.
- Utilize electronic tools like EMR to efficiently document assessments, clinical findings, and treatment plans.
- Observe, record, and report patient conditions and reactions to treatments to physicians.
- Document assessments, clinical findings, and treatment plans using EMR.
- Respond to/refers incoming patient treatment-related phone calls.
- Instruct patients and families regarding medications and treatment instructions.
- Maintain patient confidentiality.
Requirements
- High School Graduate
- Bachelor Degree Nursing
- 2 or more years of RN experience
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
