About the role

The Ambulatory Care Nurse supports the coordination, integration, communication, implementation, and evaluation of nursing practice and clinical policies. This role involves participating in quality and performance improvement activities, collecting patient healthcare data, and collaborating with licensed providers to develop patient-centered plans of care. The nurse provides comprehensive care, including administration of treatments and medications, disease prevention guidance, rehabilitation, public health care, and supportive care for symptom relief.

Responsibilities

• Performs physical examinations and diagnostic tests.
• Collects and records patient health history in the electronic health record.
• Provides information, counsel, and education to patients and families regarding test results and treatment plans.
• Administers medications (including vaccinations, IV therapy, oxygen), provides wound care, and performs phone and in-person patient triage for clinical concerns.
• Assesses and monitors patient responses to treatments, procedures, and anesthesia.
• Interprets patient information and makes treatment decisions as appropriate.
• Consults with clinical supervisors and licensed providers to determine best treatment plans.
• Directs, supervises, and delegates tasks to licensed practical nurses, medical assistants, and technicians.
• Supports coordination and implementation of nursing practice and clinical policies.
• Ensures compliance with Patient Safety initiatives and key quality measures (e.g., MIPS, Care gap management).
• Participates in activities related to NYULH clinical standards and external regulations.
• Collaborates with interdisciplinary teams and promotes collegial relationships.
• Provides emergency care in accordance with organizational policy.
• Delegates nursing interventions to implement patient care plans.
• Engages in self-assessment and professional development activities.
• Executes medical regimens as prescribed by licensed providers.
• Assesses patients, identifies unmet needs, and plans comprehensive care.
• Coordinates care with other healthcare providers (e.g., respiratory therapists, social workers, dieticians).
• Provides direct care, including administration of treatments, medications, and health counseling.
• Performs onsite testing (e.g., POC lab testing, EKG, phlebotomy, spirometry, allergy skin testing).
• Performs medical and nursing treatments (e.g., wound care, suture/staple removal, urinary catheter care).
• Monitors patients pre-, intra-, and post-procedure, testing, or medication administration.
• Administers IV therapy, including blood products, fluids, and chemotherapy (with certification).
• Manages clinical in-baskets and relays critical lab results to providers and patients.
• Performs phone and in-person triage for patient concerns, escalating as needed.
• Conducts care/case management for defined patient populations.
• Identifies patient and family health deviations and advocates for appropriate interventions.
• Develops and implements patient-centered care plans with individualized goals.
• Educates patients and families to promote healthy adjustment to health changes.
• Evaluates patient progress toward expected outcomes.
• Serves as an NYU Langone Health Faculty Group Practice Brand Ambassador, upholding mission and values.
• Applies the FGP Service Strategy (CARES: Connect, Align, Respond, Ensure, Sign-Off) in patient interactions.
• Proactively anticipates patient needs and participates in service recovery using the LEARN model (Listen, Empathize, Apologize, Resolve, Notify).
• Collaborates with Patient Access Center and Central Billing Office to enhance patient experience.

Requirements

• Current Registered Professional Nurse license in New York.
• If hired after 9/1/25: Bachelor’s degree in Nursing from an accredited institution required; Associate Degree in Nursing may be approved contingent upon Nursing Leadership approval, with a minimum of two years of nursing experience.
