Ambulatory Care Coordinator

About the role

Baptist Medical Center is seeking an Ambulatory Care Coordinator to join their team in Jacksonville, FL. This is a Full-Time days position working Monday through Friday at Baptist Metro Square.

Responsibilities

• Collaborate with patient and their families to facilitate discussions around advance care planning.
• Engage with patients to understand their end-of-life goals and preferences.
• Work closely with the healthcare team to develop a personalized plan of care that aligns with the patient's wishes and medical needs.
• Maintain a compassionate and patient-centered approach, providing emotional support and guidance to patients and their families during what can be a challenging time.
• Accurate documentation.

Requirements

• 1-2 years Case Management
• 1-3 years RN Hospital experience
• Associate's Degree in Nursing (ASN)
• Basic Life Support (BLS)
